By appearing in this interview, I grant The Divine Clinic Foundation, its affiliates, successors, and assigns a perpetual, irrevocable, royalty-free, worldwide license to record, edit, distribute, and otherwise use my name, image, likeness, voice, performance, and contributions (“Material”) in any format or medium for any purpose, including promotional, educational, commercial, or testimonial use. I waive any right to inspect or approve finished recordings or their use, and understand that the Material may be used alone or in conjunction with other footage, text, or graphics.

I further waive any right to compensation, claim, or remuneration of any kind arising from the use of the Material, and release The Divine Clinic Foundation from all liability, claims, or causes of action relating to its use, distribution, or reproduction in any context. I acknowledge that I have read this waiver prior to participation, understand its content, and sign it voluntarily and without expectation of payment.

I also acknowledge that any statements or testimonials I provide are expressions of my own present sensory impressions and personal experiences. I understand and agree that none of these testimonials or statements have been evaluated by the U.S. Food and Drug Administration (FDA). This program represents faith-based healing practices and is offered to the public as an alternative approach to care, complementing the emerging acceptance within modern medicine of natural and holistic methods of body healing.
